Please complete this request and we will contact you as soon as we can. Our staff and practitioners work only certain days of the week. In addition due to the busy schedules and the requirement for our practitioners to review and accept the referral, it can take up to 1 week to respond. If you are from an organisation filling this form on behalf of a client, please specify this in the notes. Please see our appointment process here.

Please note we do require a referral from GP or specialist attached. We can accept named referral addressed to our psychiatrist, referral to any other psychiatrist or open referrals made out to 'any psychiatrist'.
